Please review the following statements and select the checkbox to indicate you have reviewed and understand each one. 

I understand that funding for this opportunity has been prioritized for one caregiver and one child with Down syndrome, so that as many children with Down syndrome as possible are able to participate. We will accommodate an additional sibling and/or an additional parent/caregiver, with a maximum of 4 family members per registration. 

I understand that if I am bringing an additional parent/caregiver or a sibling that I will include them in the registration below. 

I understand that this event is designed to include a parent/caregiver, and therefore he or she will need to provide supervision to ensure the safety and participation of their child. 

I understand that due to limited capacity, families will be provided this opportunity based on the sequence of registration (first come, first served), and that some families may be informed they are on a waiting list. 

I understand that if anyone in my family that I have registered can no longer attend, then we will immediately inform Down Syndrome Innovations (via morgan@kcdsi.org) and remove our registration to ensure other families have an opportunity to attend.
